Registered Nurse (RN) – Skilled Home Health
Position: Registered Nurse (RN)
Reports To: Director of Nursing

About the Role
We are seeking a licensed Florida RN to deliver skilled nursing care to patients in their homes, serve as clinical case manager, and supervise ancillary care staff in compliance with AHCA Rule 59A-8.0095.
Minimum Requirements
- Current Florida RN license per Chapter 464, F.S.
- Level 2 background screening clearance
- Current CPR certification
- Valid Florida driver's license and reliable transportation
- Home health or clinical nursing experience preferred
Key Responsibilities
- Serve as case manager for all nursing and nursing/therapy patient cases
- Maintain clinical records for each patient receiving nursing care
- Report patient condition changes or deviations from the plan of care to the supervising physician
- Assign and supervise personal care tasks to LPNs, CNAs, and home health aides
- Conduct supervisory visits of unlicensed assistive personnel as required
- Participate in the agency on-call rotation to ensure 24-hour patient access to nursing staff
- Provide care within the full scope of practice authorized by Florida RN licensure

Physical Therapist (PT) – Skilled Home Health
Position: Physical Therapist (PT)
Reports To: Director of Nursing / Administrator

About the Role
We are seeking a Florida-licensed Physical Therapist to provide skilled physical therapy services to patients in their homes as part of a physician-ordered plan of care, in compliance with AHCA regulations and Florida Statutes.
Minimum Requirements
- Current Florida Physical Therapist license (Chapter 486, F.S.)
- Level 2 background screening clearance
- Current CPR certification
- Valid Florida driver's license and reliable transportation
- Home health or outpatient therapy experience preferred
Key Responsibilities
- Evaluate patient functional status and develop individualized physical therapy plans of care
- Provide skilled PT services including therapeutic exercise, gait training, and functional mobility
- Coordinate care with the RN case manager and physician
- Document all visits, progress, and patient outcomes in the clinical record
- Supervise Physical Therapist Assistants (PTA) as applicable
- Participate in discharge planning and patient/caregiver education
- Comply with all AHCA and Medicare/Medicaid documentation standards

Occupational Therapist (OT) – Skilled Home Health

About the Role
We are seeking a Florida-licensed Occupational Therapist to provide skilled OT services to patients in their homes as part of a physician-ordered plan of care, in compliance with AHCA regulations and Florida Statutes.
Minimum Requirements
- Current Florida Occupational Therapist license (Chapter 468, F.S.)
- National Board for Certification in Occupational Therapy (NBCOT) certification
- Level 2 background screening clearance
- Current CPR certification
- Valid Florida driver's license and reliable transportation
- Home health experience preferred
Key Responsibilities
- Evaluate patient functional abilities in activities of daily living (ADLs) and instrumental ADLs
- Develop and implement individualized occupational therapy plans of care
- Provide skilled OT interventions including adaptive techniques, home safety evaluations, and durable medical equipment recommendations
- Coordinate care with the RN case manager, physician, and therapy team
- Document all evaluations, visits, and patient outcomes per AHCA and Medicare/Medicaid standards
- Supervise Certified Occupational Therapy Assistants (COTA) as applicable
- Educate patients and families on home programs and safety modifications

Certified Nursing Assistant (CNA) – Skilled Home Health
Position: Certified Nursing Assistant (CNA)
Reports To: Registered Nurse (RN) Supervisor

About the Role
We are seeking a Florida-certified CNA to provide personal care and assigned nursing tasks to patients in their homes under RN supervision, in compliance with AHCA Rule 59A-8.0095 and Florida Statutes.
Minimum Requirements
- Current Florida CNA certification on the Florida Nurse Aide Registry
- Level 2 background screening clearance
- Current CPR certification
- Valid Florida driver's license and reliable transportation
- Home health or long-term care experience preferred
Key Responsibilities
- Provide personal care tasks as assigned by the supervising RN (bathing, grooming, toileting, dressing, ambulation)
- Assist with self-administration of medications if trained and authorized per agency policy
- Perform delegated nursing tasks as authorized by the RN per AHCA rules
- Report all changes in patient condition or safety concerns to the supervising RN immediately
- Maintain thorough and accurate documentation of all care provided during each visit
- Participate in required supervisory visits with the RN
- Follow all agency infection control, universal precautions, and patient safety protocols
