About the Role

We are a Florida-licensed skilled home health agency seeking a qualified Director of Nursing to oversee all clinical operations in compliance with AHCA Rule 59A-8.0095 and Florida Statutes Chapter 400.

Minimum Requirements

  • Current Florida Registered Nurse (RN) license per Chapter 464, F.S.
  • Meets criteria defined in Sections 400.462(10) and 400.476(2), F.S.
  • Level 2 background screening clearance (Section 408.809, F.S.)

Key Responsibilities

Registered Nurse (RN) – Skilled Home Health

Job Title: Medical Social Worker (MSW) – Home Health

Position Summary

FMC Home Health Services LLC is seeking a qualified Medical Social Worker (MSW) to provide medically necessary social work services to patients in their homes. The Social Worker will assess psychosocial needs, develop care plans, and coordinate community resources in compliance with federal and state regulations.

Key Responsibilities

  • Conduct initial and ongoing psychosocial assessments of patients and families
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team
  • Provide counseling, crisis intervention, and emotional support
  • Assist patients with community resource coordination (financial assistance, housing, transportation, etc.)
  • Participate in care conferences and case management meetings
